Registration Certificate of the establishment (any one):

Shops & Establishment Act Registration

Factory Act License

GST Registration Certificate (optional, if available)

PAN card of the business or company

Certificate of Incorporation (for companies or LLPs)

📍 2. Address Proof of Establishment
Utility bill (electricity/water/telephone)
OR

Rent agreement with NOC from the landlord
OR

Property tax receipt

👥 3. Employee Details
List of employees with:

Full name

Father’s name

Date of birth

Date of joining

Aadhaar number

Salary details

Number of employees currently working

🏦 4. Bank Details
Cancelled cheque
OR

Bank passbook copy with IFSC and account number

🧑‍💼 5. Owner/Employer KYC Documents
PAN card of employer/director/partner/proprietor

Aadhaar card

Passport-size photograph (sometimes requested)

📜 6. Digital Signature (Optional but Useful)
Especially useful for signing returns and documents on the ESIC portal
