ESIC Registration Form (Form-1)
📌 Purpose:
Form-1 is the application form that employers must fill out to register their establishment under the ESI Scheme on the official ESIC portal.

✅ How to Access Form-1
Option 1: Online (Preferred)
Visit: https://www.esic.in

Click on “Employer Login”

Register and log in

Navigate to "New Employer Registration"

Fill Form-1 online directly on the portal

Option 2: Downloadable PDF (for reference only)
A sample or draft version of Form-1 can be downloaded from:

Employer Details Name, PAN, contact info
Factory/Branch Details Address, nature of work, number of employees
Date of Setup Date of commencement of business
Banking Info Bank name, branch, account number
Employee Info Total number of employees, salary breakup
Registration Info Factory Act / Shops Act / GST number (if any)

List of employees with details

You will receive a 17-digit ESIC Registration Number

This number is used for all future filings, compliance, and employee registrations
